Any of those could work. The trick is around people with titles, different last names and how formal you wish to be. All the wedding sites online have guides for addressing invites and/or an etiquette book could help you out. What we put depended on the couple.

It depends on how traditional you are. Mr. and Mrs. Mark Smith is most traditional, but some women are offended by it. You don't strike me as overly traditional, so I would choose Mr. Mark Smith and Mrs. Amy Smith.

I would go with #1 or #2. I think #3 has too many words. Unless of course you are sending it to a non married couple.

I'd go with #1. That way if they have different surnames is just

Mr. Smith & Ms. Jones

Not too long, and just formal enough

I addressed my save the dates informally and went with #2. I saved the formal addresses for the actual invitations
