Adding a day to tickets

Yes, though I don't understand what you mean by "activating your 5th day".

Right.
You can upgrade a ticket as late as up the end-of-business on the day that you use that ticket's last asset.

So, you can actually upgrade a ticket that has been "all used up" that last day.
 

Oh I thought in order to add a day there had to be at least 1 full day left on the ticket, that is why I said before activating the 5th day. So we could be in the park on our 5th day and add a day while we are there?

Yup.

Otherwise, guests who were IN a WDW park using a one-day ticket would not be allowed to add a day or more to their trips… and Disney would love for them to add days to their trips.
 
It has been advised to use the first day of the ticket to get through the turnstiles and then add the other day(s) as needed. Maybe that's where the one day thing came into play.

Its more that tickets not bought as part of a package from say from UT have this rule. It was added a few years ago. It was to curb ticket re-sellers that would buy used up tickets as people are leaving the area and then add days to them in order to sell them to someone else.

Ok so we have 8 day hoppers with wp&m on a 9 day stay planning 1 off day during the week and visiting parks on arrival and departure day
so we change or mind and go to a park on or off day and we don't want to decide until our actual departure day as long as we have a wp&m option left and are in the 14 days we can still add another park day

Yes.

But, you could even add the extra park day if you were already USING the last WP&M ticket on that last day.
