A somewhat embarrassing question…

As most people have said Castaway is pretty lizard free. I don't think I've ever seen one but we stick to pretty populated areas. Cozumel I distinctly remember seeing them on a Jeep excursion but they took us pretty far off the beaten path to ruins and they kept to themselves. They didn't seem interested in us. I don't remember seeing them near the port either.
 
There are small anoles and geckos on CC. They are mostly in the foliage of the island, and you are unlikely to see them. They may come out to sun themselves on the paved areas but will run away if you get close. Iguanas do not commonly live in the that part of the Bahamas, and I have never heard of an iguana sighting on CC.

Anoles and geckos live on Cozumel too and are likely to run away. You will not see iguanas on the dock area where the cruise ships dock. Where do you meet your snorkel tour? Is it one that visits the San Gervasio ruins, Castillo Real or Punta Sur ecological park? You might see iguanas in those locations.

Thank you! We are booking a snorkel tour with a local company (still looking at a few options) but it appears we’d take a 5-10 min cab to a marina or their business as the meetup spot and spend the rest of the excursion in the ocean. I guess unless they were on the dock as we excited the cruise, headed to the taxi or boarding the excursion boat I wouldn’t see them.
 
Understand your phobia and appreciate your honesty. I am afraid of heights-so when I see those para sailing, brave folks-lol. I did see some small lizards/geckos? on castaway last week. They were mostly in the foliage along the walking paths
 

I agree with everyone else (and congrats on positive replies all!), phobias are real and everyone has something they fear. Continue with your therapy and perhaps visualize them all on their own vacations - maybe wearing mickey ears and being more afraid of you than you are of them. Don't let it hold you back, you are stronger than you know.
 
In Cozumel, we did the Fury catamaran snorkel and beach excursion, and I don't recall seeing any lizards. On Castaway Cay, as others have said, there are small lizards (2-3 inches) in the foliage. I saw them on the bike path, but they were so skittish that I could hardly get close enough to take a picture. I don't think I've ever noticed them in the more populated areas like the beaches, shops, or restaurants.
 
We’re going on a 5 night Western Caribbean cruise from Miami in January with stops at CC and Cozumel and I have a somewhat embarrassing question, so please don’t laugh.

I’m scared of lizards, yes, even the really small ones, and was curious what to expect on CC and Cozumel. I’d assume the ones on CC would be relatively small but was curious if there are areas I should avoid if I don’t want to see them. I know in Cozumel there are iguanas. We are looking at booking a snorkel tour and I wasn’t sure how many I’ll see leaving the ship and headed to our snorkel excursion.

I just want to let you know we were on CC in August on Serenity Bay in Cabana #23, and there were 2 decent size lizards that kept bothering us inside the cabana. I was up on the couch screaming and while my husband was getting rid of them!! I only tell you this, not to scare you, but so you can be on guard! Fear is no laughing matter!

We definitely have seen (small, maybe geckos?) lizards on CC. Like others have said, with no phobia, I don't recall specifics - but I do recall my kids pointing them out. They were usually along the pathways near foliage. We didn't have any crawling on the tables at Cookies or on the beach loungers. None of them seemed to have any desire to hang out with us!
