The Amazon default setting does NOT require a PIN: you have to opt into that restriction. You can buy your heart out with nothing to slow you down if you don't set it.
Probably 90% of the time when you get an unexpected Prime Video charge, it happened because of a child who had access to an account-enabled device. Also, sometimes people will click on the "buy" button by mistake when they mean to click a different action, especially when using a TV remote, and it bills, even if the user backed out a few seconds later.
