90 day Recovery Letter

My friend is having a hard time getting a recovery letter from Kaiser. Would anyone be willing to post a redacted letter that she could show her Dr?

I am having a hard time also. My doctor sent one but it doesn’t have my birth date and also specifically state “ clear for travel”. Also I am new to Kaiser and not sure how to upload my positive Covid test. I would really appreciate any help also!
 

If you go to the Kaiser website and look at your test results, there is an option for "print". If you click that it will pop up a version that is formatted for printing, and that version will have the Date of Birth on it (unlike the web page version). You should be able to print that out as a pdf and upload that to Safe Passage.

First, my provider did not provide the test, I just had to send him a copy of the test results. My negative test results mentioned in the letter were personal tests I took at home unproctored

Ms. XXX is a patient of mine who recently had COVID-19 infection diagnosed on July 28. At this point, her symptoms have completely resolved, and she has fully recovered without any residual symptoms. She has tested negative repeatedly for any persisting COVID-19 infection. At this point, she is free of any COVID-19 infection, and is free to travel without limitations.
Please feel free to contact me if I can provide any further information for this patient.

Hope this helps. I have seen people reference that they needed to pay a copay and/or have a telehealth appointment to get the letter

Thank you all for your help. I tested positive on Monday for a home test, and waiting for Kaiser to post the results. I will send a copy of your letter to my Dr to show her what I need.
