For those who shop at PetSmart... On my register receipt today was an invitation to complete a survey and get a $5 check in 4-6 weeks. The survey was automated (you have to call an 800 number, can't do it online) and took about 5 minutes to get through. Then you give your name, etc. and a check will arrive later. They say upfront that they will keep information confidential and it's just for research purposes -- never to be given away or sold.
I don't know if this is nationwide or if all receipts get the invitation or if you have to buy certain things to trigger it to come out (we bought dog treats). But if you shop there anyway, you might want to run by and see if you can get the invite. The invite says the promotion ends 1/16/05.
