I did it last fall. Unfortunately, most of both the information I used for 4x6 cards last year and the information I'm using this year for my 8.5x11 (one sheet per park) is either proprietary or very specific to my family. I did them in a mix of Pages (part of iWork for the mac) and neoffice J (a mac version of openoffice). It really isn't that hard to do though, even in word. Set up a textbox that's 4x6 and fill it with the stuff you care about. We fiddled around with layouts a lot until we got something that we really liked.
Last year we had....
Best parks by day of the week color coded (from TGM, with park hours and parades/fireworks times)
ADRs (with conf numbers and restaurant phone numbers)
A list of quiet/out of the way places to stop and rest with tired toddlers.
A list of preferred counter service locations
This year I'm doing
best parks by day of the week (using a combo of touringplans.com Best days and my own logic scheme) color coded with park hours and parades/fireworks
ADRs without conf numbers (as the tourguide I'll keep a copy of the ADRs with conf numbers buried in a pocket, but they were pretty clutttery last time)
A different set of out of the way places to sit, since the TGM list was close to useless (this one is based on where we actually saw handy places to let the kids take a break)
A spot to write down where we parked
A list of places to eat that the picky eater has pre-approved and what they will eat there.
A map on the reverse (from the passporter, highlighted with our points of interest)
A list of things that various people particularly want to do in the parks.
To make it disnified I used fonts like marker, walt disney script and mousebats. This year I'm going more for legible than disnified.
