1609 - how would John Smith address a letter back home?

DS has to pretend he is John Smith writing a letter back home describing the difficulties he is having with the Jamestown settlement. He is writing his letter to the King James I but how would it begin? Not "Dear"?
And would it end Sincerely, John Smith?
 
This is just off the top of my head, but I would think he would start it with "Your Majesty" and end with "Your humble servant, John Smith".....or something like that. Hope it helps :D
