After you've registered yourself there's a field on the home screen to register your product. Enter your Wii Fit Plus code and (I forget if it's before or after the short survey) it'll ask you if you want the tote. I think the offer is in a popup so make sure you have your popup blocker turned off for the minute it takes to do this. I wish could go back and do it again to tell you for sure.
