where to send a gift for a CM ?

Hi,

I hope someone can help me with this. We met a wonderful CM at EPCOT last august and she was great to my family and I and now, we would like to send her a thank you gift but I don't know where I could send it to and make sure that it reaches her.

Anybody here knows how I should proceed ?
