Where to order free Disney maps?

I know I've seen it before , but can't find it. Does anyone have the address/website for ordering the free personalized Disney maps?

you can go to www.disneyworld.com, but i think i heard somewhere that they won't mail to Canada, maybe someone else can confirm that
