Where Do I Find the Form?

I need to fill out a Guest Services Form for my mom's allergy. Can I find this online somewhere? I searched for it on the DCL website but couldn't find it.

They don't have an allergy form anymore. You need to inform them via phone or when you arrive on the ship.

What is the Special Services Information Form and how do I complete it?

This optional form allows you to report special needs. It also allows you to arrange for wheelchair access and/or lift, oxygen and/or special dietary needs so that Disney Cruise Line can best accommodate the members of your travel party during your vacation. You can download the page in a PDF file, print it and fill it out. Then you can mail, email or fax the form, which includes the mailing address, email and fax number.

Special Dietary Requests:
Disney Cruise Line can accommodate some special dietary needs, including providing no sugar added, vegetarian, low fat, low sodium, lactose free and kosher meals at no additional charge. Food-related requests will only be provided in table-service restaurants, and not at quick service locations or through room service. Any food allergies, intolerances or dietary requests should be brought to the attention of your Head Server prior to ordering.Unfortunately, Disney Cruise Line may not be able to accommodate all special dietary requests.

Thank you Minniemiss for the link. I had called DCL the other night and told the girl that I wanted to inform them of my mom's food allergy. She was trying to enter it, but she said that there wasn't an option for garlic. She told me to fill out the Guest Services Form. The form that you gave me in the link doesn't have a section to fill out for allergies.
