Wheelchairs at Disney?

Hi guys! I have a Disney trip planned in November and one of my family members coming with me can not stand for long amounts of time because of a nerve ending disease she has. We are planning on getting her a wheelchair but I am just concerned because I have never been to Disney before with having someone in a wheelchair. Does anyone have previous experience with it? How they transfer people to the actual ride, if there is a list of rides wheelchair bound people can go on, etc.?
