What exactly is included in the QS meal

So I understand the adult QS meal comes with an entree, drink, and dessert. What I'm curious about is what is included within those? For instance if I'm at my resort QS counter is an entree just a burger? Or can I get a side like fries or a salad with it? Is the drink just a soft drink? Of if I'm at a place that makes shakes can I get a shake as my drink?
 
One entree or combo meal, one non-alcoholic beverage, and one dessert. No separate sides. If the fries come with the burger, it's a combo meal and they are included. If the fries are sold and priced separately, they are not included. There are some combo meals in a few places (Pizza Planet is one) that do include the salad as part of the combo. But the general rule is, if the side is priced separately from the entree it is not included. You may be able to use a snack credit to add some side dishes.

Breakfast does not include the dessert. No substitutes, you just give up the dessert allotment if you use a QS credit for breakfast.

If the CS serves milkshakes (not a whole lot of them do), they MAY allow it as a beverage. It's up to the restaurant. Coffee, tea, soft drinks, milk or bottled water are usually permitted as beverages.

1. Lunch and dinner include entree, non-alc fountain beverage and dessert. Breakfast does not have a dessert component - just the entree and bev.

2. When you read the menu "entree" includes everything in the description of that entree.

3. Sides are only included if it is part of the entree description. If there are no sides, but they are elsewhere on the menu with a separate price point you can order it, but you'll have to pay OOP for it or use a snack credit if there is the purple dining plan logo next to it.

4. Standard non-alcoholic fountain drinks such as coffee, tea, iced tea, soda - basically the kinds of beverages that come with free refills at many restaurants. Also, at CS a bottle of water. In fact, many CS will let you have an extra bottle of water in place of the dessert at lunch or dinner.

5. Shakes are not automatically included, but some restaurants might let you choose it. This is more likely at Disney owned table service restaurants.
 
It may well have changed in the 3.5 years since we went, but the QS at POFQ, POR, AKL, and Wolfgang Puck all encouraged us to get a second beverage (e.g. coffee AND orange juice, or coffee AND a bottle of water) or a piece of fruit for "dessert" at breakfast.
 

The second beverage for breakfast was discontinued in 2011 or 2012, can't remember which, but it's no longer done.
