This probably isn't too helpful, but I found someone on Ebay to make them. Here's pictures of how they turned out.
The front had our monogram (Sorry this is the best picture of the front)

The back had all the information

I'm only making 10 bags, 1 per family.

I haven't made them yet. The tag is also going to a simple 3x2 with my wedding colors. and the back will have the name of the family, resort, check in date, and reservation number. These will also be handmade.
