I was told on here that the QS restaurants will give free cups of ice water. First how big are the cups and 2nd if you bring your own cup will they fill that cup with ice water?

Most places we got the same size as the regular soda (16 or 22 oz, maybe?) but a couple places I think it was a bit smaller, but still at least 12 oz, I think. I do not think they will fill your own cup or bottle, you have to pour theirs into yours.

The above is correct.

You can ask for a larger water.
 
A few venues don't have a soda machine to dispense water. But they will give you a cup of ice if you like to take to a water fountain and fill with water.
 

During our trip in September, we asked for a large cup of ice water at many, many QS locations and they had no problem giving them to us. Some of the QS locations had the ice water pre-made since it was really warm and they were always the medium to large size and that was really nice.
On a couple of days, we carried our water bottles and we would get a large cup of ice water and just pour it into our water bottles since they were easier to carry and store if we didn't drink it all.
 
We were there over 4th of July and likewise, there were several places that had them ready to go. We were always given the full sized fountain drink size cup. We always got two (there were 3 of us), we would drink one then pour the second into our Brita bottle.
 
I can agree that you will not get your own personal bottle of any kind refilled with water at any Disney park location. Years ago, Animal Kingdom had a personal mug of soda you could buy (no, it is NOT there anymore so don't say it is :)) and then get refilled during the day. My mother-in-law's sister was so happy with this so she bought one and just loved the design. When she went to get it refilled, the server handed her a paper/plastic extra large drink. The sister was outraged! She wanted her drink in her mug! She was very loud. The server explained that the refill needed to be poured into the sister's mug by the sister herself. The sister screamed, "WHY?!" The server replied, very calmly, that there was no way for the Disney Cast Members to know where that refillable mug had been in the time the sister bought it and when she brought it back.
 
Sometimes they have a bunch of the small cups set up ahead of time, but you can always ask for a larger one or just get several. They will happily oblige.

It's a health department requirement. Once something goes across the counter, it cannot go back.

I cannot speak for filling your own cups. I believe, last year, we received 22oz cups of water.
 
We did this at several locations. Also, some quick service locations have the soda machine that you get your own soda and it has a button for cold water. We have refilled our big reusable water bottles with ice & water this way. This way the water doesn't have a funny taste because its gone through the filtration system.
