visa and visa waiver

I am traveling to orlando using a visa,my 13 year old son will be with me, so do I still have to fill in a visa waiver form on the plane for him or will my visa cover him as well? thanks all

He will need his own waiver form, and if you both have the same surname just one of the white customs forms.
