1: Yes, use them first but only if the 5-day tickets were purchased at a discount.
2: The anniversary will be the date of first use.
3: Yes/no. You'll receive a new RFID card then and there, but you'll need to order your AP MB via your MDE account.

1. Use your tickets first if they are resort package tickets, or tickets purchased from a third party reseller such as AAA, UCT, etc, or tickets purchased from The Disney Store. These are all tickets that Disney sold wholesale to those entities, and are qualified for price-bridging if used at least once. Tickets from Disney directly, such as online, mail order, or from a Disney ticket outlet, are not qualified for price-bridging.
2. If you want to get full credit for your 5-day MYW tickets towards your AP, then your AP will start on the first date you used your 5-day tickets. Otherwise, your 365-day AP would be 370 days (365 days plus the 5 days of your original ticket that you got full credit for).
3. Follow the answers from PP for new AP bands.

Just want to point out to others with non discounted tickets that you can use your regular MYW tix and then upgrade them the last day they are valid too. My friend decided to put the $257 two day park hopper she had gotten towards an AP so that she didn't have to start over again next time she comes down. Once the the ticket is used and expires, that $ is no longer able to go towards upgrading an AP. Good choice! We even hit an extra park night once we all had passes! Happiness! (And yes, your anniversary is the first day you started the ticket regardless.)

Just want to point out to others

with non discounted tickets that you can use your regular MYW tix and then upgrade them the last day they are valid too.

Whether a guest has a discounted ticket or a non-discounted ticket, that will be the case.

Guests can upgrade a stand-alone ticket as late as on the same day that the ticket's last asset is used, and still get the full current Disney gate-price as a new ticket with those same assets in the upgrade transaction.
