Under 5 and Safe Travels

My youngest will be under 5 when we travel. I have added her to my Safe Travels account, but do I need to apply for an exemption for her or is it automatic? I know she doesn’t need to test, and I assume we’ll have to fill out the health form, but I want to make sure I’m doing this properly.

Also - do we need to bring proof of age? She’s almost 4, but tall for her age and acts as bit older thanks to 2 older sisters
