Turf Club invite

I am almost ready for our trip....... the only thing I don't have & can't seem to find is a blank Turf Club invite?! I have found blanks for all the others & personalized them myself but am stuck ont his one. Does someone have one & I am just not seeing it? Or is there someone who would do one for me? You can give me a blank - that's fine. Otherwise the info is :
