So I called for an appointment for DW's surprise trip and they said there is a 20% service charge. Is that a tip? I asked her and she said not really. She said that it goes to all the staff and not only to the CM's performing the service. If we tip that would go just to her CM's. Not sure how to read that one. They said they are booking a ton of these and filling up.

Like Connie B noted the tip should be based on the full price, not a discounted price. That is just basic tipping etiquette.

I think what Nonsuch means is that the spa's automatically added amount is calculated from the discounted price for this deal, not that you shouldn't add more to it.

This is a great deal and in our timing... but I've been spoiled by Coldwater Creek the Spa here and I rarely does anything live up (not even the cruise line spa!).

They probably do that, and at the 20% rate so people don't undertip to start with.

In any case whenever you receive something at a discount or complimentary that is a commonly tipped for, it is polite to tip at a rate adjusted for the regular full cost.

I believe in tipping well, but I also still want this to be a value. I will tip the 20% on the actual cost, but I don't know if I should figure in the service charge or not.

If you are concerned just ask if the service charge is going to the masseuse so you can be more confortable that everyone is getting tipped as you intend.
