Traveling with someone elses child

I haven't searched for the answer because I'm not really sure what I'm asking for :)

We're flying to Anaheim in a week (so much left to do! OMG!) and we're taking our friends daughter with us. I have a notarized letter from her parents, but I was wondering how to deal with customs and the forms that are usually one per household? Anyone dealt with this, or know the proper terminology so I can google it :)

I believe we filled out a separate one for the guest and all walked up together and explained the situation, showed the note and everyone was happy.
 
Ditto. We took her up with us but used a separate sheet for her declaration after all she was part of our travelling party just not our household.

Yep, us too. Separate form, all go to the counter together with the letter out. No problems.
 
Yes this is what my daughter did when traveling with her grandmother.
