Traveling with my daughter's friend

We are taking my daughter and her friend on a cruise in January to celebrate my DD's 13th birthday. Her friend has her passport, I will get a copy of her health insurance card, and I'm purchasing travel insurance. Does anyone have a list they've used to make sure I have what I need for her? Medical waiver, etc! Any other advice or things I might not be thinking about? TIA
 
Need a dcl form signed and notarized by both her parents authorizing you to take her.
 
I am in a similar situation as we are taking my niece with us. The only minor authorization form I can find on the DCL site does not require notarization. Is this the only completed form that I need?

Yes that is it. I am surprised there is no notary. Call DCL to be sure. Anyone could forge it with no notary
 

We have taken my daughter's friends several times. I make sure to have the cruise line's authorization form completed and I also have another form that authorizes her to travel with us by plane, ship, etc. and gives DH and I permission to authorize medical treatment for her. I get that one signed and notarized.

If you are including her on your travel insurance, just make sure your policy will cover non family members - not all of them do.

By the way, we have never been asked for the authorizations from any of the cruiselines but we did get questioned once by TSA at the airport.
 
I took my niece on one of our cruises. The only form I absolutely had to have was the minor authorization form from DCL. It does not have to be notarized, but my brother had it notarized anyway. In addition, I asked for a notarized letter from my brother and his wife stating that I had their permission to take my niece on the cruise and to get medical treatment for her. I didn't have to have that letter but I felt better having it just in case I was questioned on anything. I'm always in the "better to have it and not need it than to need it and not have it" camp.
 
Years ago, there was a form to have filled out and notarized on the DCL site if you were taking a child that was not yours. I had my mom complete and notarize it for my younger sister who we took with us. However, we were never asked to show it at any point leading up to or on the cruise. She was 15 or 16 at the time, and this was about 7 or 8 years ago.
