Transferring reservation to Costco Canada

I am hoping there are some Canadians that can help me out with this question. I have a reservation that I booked directly with Disney a few days ago. I would like to transfer it to Costco. I am in Canada so I am unsure what to put on the transfer form. Is there a Canadian address I should be using for Costco Canada? Or does it all go through the US Costco travel? Thanks!
 
Here is a copy of an email from Costco Travel Canada detailing what to do. I’ll attach the transfer form to fill out, you must do it within 30 days of making the booking.

“Dear Costco Traveler,
Attached is the Disney transfer request form. If the booking was made more than 30 days agoincluding a placeholder reservation or is paid in full, Disney will not allow the booking to be transferred to Costco Travel. For reservations held without payment, the 30 day clock starts at the time to booking is created, and not when the deposit was made. A placeholder reservation is when the member has put money down on a future cruise, but has not picked the ship or sail date, yet.
If the booking or placeholder reservation was made less than 30 days ago and is not paid in full, please complete the box on the left side of the form; Print and sign your name below the box, date the form and return to Costco Travel.Please complete one transfer request form per reservation and the passenger who signs the form must be in the reservation. If you are returning the form via email, please ensure it is attached to your email being sent to Cruise.Questions@costcotravel.com or you can fax the form to 425-657-1880 Attention: Cruise Follow Up.
After you have returned the form, please wait 2 – 3 days for Disney to process the transfer request. After 72 hours has elapsed, please call Costco Travel at 877-849-2730 and a Costco cruise agent will complete the transfer request, review the booking with you and go over Terms & Conditions.
Thank you for choosing Costco Travel and we wish you a wonderful cruise vacation.”
