Template for Water Bottle Labels?

My friend who is getting married in March is looking for the template for the water bottle labels. I will eventually need it, too (as soon as we decide on artwork and colors), so I was hoping someone could help now.

Thank you, Caryn! I can't believe the minimum order is for 400 labels and the cost seems high. I guess I was overly optimistic to think I could just buy some labels at staples and print them!

Has anyone just done the labels on some ordinary label form?

I bought my labels at labels by the sheet. They were waterproof and there was no minimum! You can choose to have then printed or print them yourself, we opted for home printing. They were very easy.
