Specialty Celebration Cakes

When restaurants in Disney say that "Specialty celebration cakes are available for an additional charge and should be ordered 48 hours in advance," what kind of cakes are they talking about? Do I dream up a cool cake and ask them to make it? That seems like they'd have tons and tons of on call cake bakers then. Has anyone done this? Any pictures?

I am trying to do this as well for my son's birthday. If you do a search for "birthday cakes" you will find a number of useful threads. Apparently, once you have a TS reservation, you call the bakery and arrange it. I'm told the bakery number is (407) 939-5175.
