My sister has cancer and is in a lot of pain (takes morphine) and is tired and sort of in a fog. She is also fiercely independent and insists on renting an ecv to ride herself. I am concerned that there is no way she will be able to navigate the elevators at pop. Is there a sure way to guarantee that she will get a ground floor room. Also she is staying one last time with her little grandchildren and HAS to have connecting rooms. This trip just has to be perfect for her.
 
You need to call WDW special services, they can make this happen. I'm more concerned with her hitting someone else in a park. If she's not able to navigate an elevator, how is she going to navigate crowds?

We tried to get her a wheelchair but she insists except for epcot and the long treks to the park she will park her ecv. In any case, she will be surrounded by a ton of family so she will only hit us!
 

I believe if a second room is to be occupied by kids under 18, they will be sure you have connecting rooms.
 
As ducklite said, call WDW Special Services. They are the only ones who can guarantee everything you need.
