Should I tip if so, how much?

I have to chime in here having worked in food service myself way back when. The 15% is likely a fee for gas, transportation, etc. Definitely tip the waitress for what she is doing since she is likely losing money waiting on tables. I don't care if she is the manager's sister. That 15% is likely going to the restaurant not to the people doing the work. And like a PP said, tip her when she walks in the door. I know if I was handed a tip right away, I'd definitely go out of my way to please the client above and beyond what's expected.
 
My German Grandfather, a wise man who lived to be 89, always tipped ahead. He would say something like "Here's a little something for you for the excellent service I know you are going to give me"! I like this approach at the car wash, carpet cleaners, etc. I haven't tried it with a waitress or waiter.
