should I be hesitant to book USAir 11 months out?

By the first quarter of 2005 would be the time to be worried. If they do go out of business your tickets would be honored by another airline if seats are available.

I am preferred with US and I am not hesitating to buy my tickets on US Airways.

I would suggest visiting www.flyertalk.com Then go to the USAirways board. There are lots of experts there that can put your mind at ease.
