Shore Excursion Request Form

I've been trying for two days to download and print the Shore Excursion Request Form, but it won't load. Is anyone else having this problem? Is there another way I can get it without waiting until they send me one?

Also, are all the times for the excursions, on DisneyCruise.com, accurate?
