Sams Club deal help????

Ok I got a mailer today that says join Sams Club, get a $40 gc to use on organic items. I have to do this online using a code on the mailer, but can't find a place to input the code when trying to sign up- any help? it's kind of frustrating

I have looked everywhere online....why would they send me a savings code I can't use to sign up? it has an online code attached to get the free gift card,but I am baffled here

Is there any sort of customer service phone number on the mailer that you could call for assistance? If not, I'd call Sam's Club directly and ask them.
