Rewards Dollar Card-pick up in park

Does anyone know the locations where you can pick up your Disney Visa rewards card once you get to the World? I tried looking on their website, but I couldn't find it.

I got mine in May from Guest Services at DTD. I think all GS locations have them.

On a related note, if you call to add points to your card are they available immediately?

OP, I have definitely seen others post that they got theirs at Guest Services in MK and DTD like Shmily1 mentioned. Not sure about the other parks.
