Are there any sit down restaurants either in the parks, in the resorts, or in DTD that are not included in the dining plan?

. Yes, charmed59 - there are a number of them. And they do NOT clip their menus to a "two point" dinner.
First, see
http://www.wdwinfo.com/disney-dining-plan-restaurants.htm#ddp2015 - then read it BACKWARDS, as a quality screen. Search for the Restaurant of interest, and see if it's on the "death list".
The first screen: Is the Restaurant Disney Owned? Due to career Management rotations, the only "Disney Owned" restaurant that does NOT take
DDP is Victoria & Albert's. While the plan is "voluntary" ? No Disney career manager will reject it, as the act would be certain DEATH at the corporate level.
Second Screen: Of the NON Disney owned restaurants (forgive me if my info is a bit dated....)
1) Shula's - TIW, no DDP
2) Il Mulino - TIW, no DDP.
3) DTD - only restaurant listed is Fulton's. No Boathouse, no Morimoto. BOTH have, however, announced upcoming participation.
4) Golden Oak - only the Outpost appears destined for doom. Actual fine restaurants don't take this.
5) OFF SITE - but very close. Bull & Bear

DDP, as a "point" system, forces fine restaurants to clip official menu offerings to the compensation level of "two points" (about $75 a plate in compensation). As Everything on the menu is offered, NOTHING on the menu may go beyond that compensation level (unless the restaurant wants to go broke, OR, they make it up on OOP customers).
