Resort change after receiving DME tags?

I decided to change resorts today for our upcoming trip (end of Nov). However, I received my DME tags in the mail right after! I think I read before there is a form you can complete to request new tags that display your correct resort ... does anyone have that link??? Thanks!

No such thing. You can call DME and tell them about the change and they should send a new packet.
If the reservation number stayed the same, just carefully cross out the old resort name and carefully write in the new resort. Don't mess with the bar code though....that holds all your info.
If the reservation number dud change, just make a new DME reservation.

You can fill out the DME request form again but although I did get an email confirmation that my info was updated they never sent new tags. I had to call and request them, but they sent them out very quickly and even sent an extra set
