Rebate forms? Where can I find them?

We finally broke down and bought a DVD player. The one we purchased is supposed to have a rebate, but the place we bought if from (Foley's) did not have the rebate forms. Is there a place to get rebate forms online? Anyone have any ideas?

Contact Foley's. If the store management can't help, maybe they can connect you to the buying offices who would have more details.
You could always try the DVD player maufacturer's website. They might have information.
