We have 11 people in our party coming to WDW. We have purchased the dining plan. At a TS meal if someone orders something extra that is not included how is that paid? Each family will have registered a credit card to their room, does it get charged to their individual card?

It will depend on how you want the bill to work. It will go on whatever bill you choose (if you split it) and it can be charged back to the room or paid by credit card at the restaurant. You just tell the servier what you want. You will be charged an automatic 18% gratuity, you can pay that the same way. Tip is not covered on the DDP (it can add up if everyone is drinking expensive alcoholic drinks).
