Had a missing code in my Nightmare Before Christmas. You have to go to Disneyrewards.com and under the MY ACCOUNT tab is the FAQ. Click on it and one of the links is for missing codes. Folow those directions. I think you need to send in the receipt and the proof of purchase with a form you print out.
