Program Assessment Fee (can't find form)

I went onto the College Program Countdown section, but I can't find this form they talk about. All I see is a link for "Payment Options/Submit Payment" and when I click there, the only link is for "For guests paying via Credit Card, please click here" and some other privacy policy link. So where is the online form?

Well when I did it I payed with my dads credit card so when I clicked on the link the form showed up for me to fill out info and everything then after you pay they ask you to print out another thing that shows that you paid and it also has some other info on it. I think that might be the form that they are talking about, its the only thing that I have that looks like a form lol. I'm not really sure what to do if you're not paying with a credit card though.

I had the same problem. So i called to the program and they told me that they are not accepting money orders at this time. They were only accepting credit cards. So i had to borrow one from my sister. You have to use a credit card to send the fee.....

Yeah, I ended up using a credit card myself, so issue solved.
