It isn't referring to family style places like 'Ohana or Garden Grill.
There are several restaurants that have prix fixe menus - meaning you can choose an app, entree and dessert from a pre-determined set of selections. It's a way for those on the regular DDP to have an appetizer included in their meal. No substitutions are allowed.
Restaurants that have/had prix fixe meals include:
-Chefs de France (L,D
-San Angel (L, D)
-Marrakesh (D, but there is an upcharge of about $5-$7)
-LeCellier (L, currently while it's still 1 credit)
-Nine Dragons (L, but no dessert listed, D)
-Captain's Grill (D)
-Flying Fish (D, but not on the dining plan)
As far as the surcharge mentioned on the 2013 DDP, no has reported back what exactly it means. Marrakesh has been adding an upcharge for their feast for several years so it's not clear if it's referring to places like Marrakesh or if additional restaurants will be adding a surcharge.
