Scroll down to the link, it says a free card and £5 extra free, I guess that's a better deal than on the cashback sites but check them all.

Quidco offers £5 cashback for a new card and the £9.95 fee is waived if you are putting more than £200 on the card when you buy.

But another vote here for the FairFX $ card, used it the last twice, and it's been great. If you need to take cash out, take out as much as the ATM will allow, as it's a standard $2 fee for each withdrawal.
Topping up is easy online, or with the iPhone app.

Renewal of the card (after 2 years or however long it is) costs a few £, but to me it's worth it.
