I actually got mine in an email from Disney Destinations. When I click the link, it gives me the option of pre-ordering at the discounted rate. I think I got the email a few weeks ago & fortunately saved it, otherwise I wouldn't have known about it. I read about it here on the DIS. I thought it was just one of those "things to do on your upcoming trip" emails.

My discount code came in the US mail from disney along with all of the other items sent for the vacation package.

And to add to the list of possibilities...

I went thru a travel agent (Small World Vacations) and had to have my TA send in all the info to photopass. We are staying on site, I believe only vacations on site are allowed/given this deal. Check stitchkingdom.com and search for the info, you can also call the photopass people directly.
