Please help with link in signature

If someone could help, I would really appreciate it. I can't seem to find this in my notes.

I would like to remove the entire "OVER 500 PICTURES & 20,000 HITS GOVE FROM PHOTOPOINT" from my signature.

And in it's place, I want it to read (in BOLD if possible): Visit my DisneyWorld scrapbook
That's the phrase I want people to click on to take them to:

Thanks so much! This time I'm printing out how to do that. I never remember which order the [ ] 's go in.
