My dd and I did this last Dec. as a part of a package type thing. But if I remember correctly it's around $25-30.

I just bought a Leave a Legacy tile last week via telephone. It's a text tile that I wanted posted as a surprise for a grand gathering that my family is taking in December (14 of us). My credit card was charged $25. Not sure if the price is different for the photo version.
