Photopass Cards?

I have read people saying they get photo cards, is this correct? I have also heard that there is not as many photos taken then at WDW.

So my questions are:

1. Is there a Photocard handed out?
2. Where can I expect to have our photos taken?
3. Where would I get the photos when we're ready to leave?
4. Do you get a disc or hard copy photos? How much would these all cost?
5. Is the Makeover photo actually included in in the Makeover price, or will we get extra photopass photos that we can purchase?
