Phineus and Ferb?? Checklist?

Can someone point me in the right direction. Someone had a design that had a Phineus and Ferb checklist. I think had some of the things like; building a rocket, fighting a mummy and going to Walt Disney World.

I would love to find it and make shirts for my kids for our trip. Thanks for any help you can offer. I tried searching, but I still can't find it!
