If during online check-in you specify that you'll be responsible for another passenger's charges and indicate credit card payment, and the other passenger specifies you as the payer when they check in, do their charges: a) accumulate in their own on-board account and get charged to your credit card when the limit is reached and/or at the end of the cruise, or b) immediately get posted to your on-board account?

The records are kept at the KTTW card level. So every thing that is charged to each person's KTTW card will be shown as a charge to that person. When all the accounts associated with the payer reach a certain level, then a charge will be put through to the credit card. At the end or any time you view your charges (or ask for your charges on the classic ships), you will get a listing for each person with a separate line for each charge and a total for their charges/credits and at the bottom will total up the activity. So, if you are asking if you will be able to see how much Johnny charged and how much Susie charged and how much you charged, yes, you would be able to see how much each person you take responsibility for has charged individually.
