Does Disney ask for a child's proof of age, such as birth certificate, when buying park tickets at WDW?

Also, if child is a niece, nephew, or grandchild is it necessary to have travel permission letter?

1. No. Disney doesn't ask parents for proof. Disney only asks parents to be honest.

2. Disney doesn't ask for one. Can't speak to airlines, etc.

if you're crossing an international border you should have a letter but other than that, no need. And walking between Epcot countries doesn't count as crossing a border. :-)
 
You would need a letter if there was a medical emergency, and also its a good idea to have one you never know.
