parent release note??

We leave on Saturday for Disney world just me and my dh going to our first food and wine and the first time using our dvc !!

My mom will be watching our kids for this long weekend that we are taking.
God forbid if something happens to one of my children do we need to write up something that states my mom has the authority to take them to the dr or hospital. Do we have to get a paper stating we give permission if so, what should it say on it and should we get it notorized?? do not know if this makes any difference but we live in NY I don't know if the laws are different or not??

I am on a mini trip and wrote a note for my MIL who is currently torturing my kids. I also have an indefinete one for my mom at home. I never get them notorized. My mom at home takes my kids to the doctors all the time, only the dermatologist wanted a parent there. Go figure. In an emergency situation of which I have had my fair share they treat first ask questions later.

We have always had one that was notarized. My parents needed to use it once in all the years and they were asked for it.

Not sure if its still needed, but I would do it just to be on the safe side.

I always leave a note giving permission for person in charge to sign for any medical treatment. Be sure to list in the note phone numbers that you can be reached at, date of birth for all children along with ages and any allergies or medical conditions they might have. Also, include a list of any medication they might be taking, the pediatrician's name, address and phone number. I would make copies of their insurance cards as well. Some of the information like the birthdays and ages you might think your paents will remember BUT in an emergency having it written down is easier and more reliable. Have a great time in Disney!!
