OT: Memorizing address and phone #

This is not for a Disney trip, it is just for life skills. I have been trying to teach my 5 yo his address and phone number for quite awhile and he is just not getting it. It took forever to teach him 911 and this is not going much better. I am looking for some kind of song or rhyme that might help him memorize it. Any ideas or suggestions?

I have done the little house cut outs with gluing the numbers on and a few other projects, but like I said, he is just not getting it.
