OT - Leaving children with sister

DH and I are going on a cruise June 12-15. The kids are staying with my sister and her husband in Orlando. She works for Disney, so they'll have a fun Disney weekend while we're gone. My question is about healthcare. Do I need to have some sort of document that allows my sister to seek medical care for my kids if necessary? If so, what do I need? Obviously I'll leave her my insurance card.

If you've left your kids, please let me know how to prepare! Thanks

I've always written a short letter that states that I authorize that person to seek medical attention for my son. Then I sign it. I also give that person a list of my son's allergies and any medical conditions (if he had any).

We always have a letter drawn up and notarized stating that my sister and her husband have rights to make decisions for medical care for our children. Also, we do give them the insuance cards.

My doctor has a document that you can fill out. So I would check with yours. & rememeber to add a least one day to the end incase you get delayed.

What others have said is great. Be sure to write down their pediatricans phone numbers too. In case of an emergency it will be helpful for the clinic in Orlando to obtain medical history. If either of the kids have allergies to meds or any conditions make sure they are noted on the release too. Have a great cruise!

I always do what others have mentioned......letter and insurance card when someone watching one of my kids.

I usually list any allergies - to foods, animals, etc., and what meds they can take and what dosage to give them. Usually I supply any OTC meds so its readily available.

Do a google search for "parent medical consent form"
There are several good ones that you can print out and use.

My DD went to Paris last year with friend and her mom. I wrote a note authorizing her to seek medical attention if necessary and had it notorized at my town hall. This is what I was told to do by my insurance carrier. Better safe than sorry.

I second the google search. It's what I did when my parent's watched my boys for a week (they do it most summers just because). I also had it notarized. Here's a link I just found - there are several others if you google.

Perfect! Thanks for all the advice and the links. I'll find a premade form to print out and use. One of the women I work with is a notary, so it will be super easy to have it notarized.

Everything listed above plus be sure she has the kids Socials in case of a hospital visit. They always need that.

This is almost perfect, but get the letter notarized. If it is notarized, it has legal effect. Otherwise it does not.
